religious education program

The St. Elizabeth Seton Religious Education Program provides religious formation and sacramental preparation to children in grades K-8 and is designed to supplement the religious formation that parents, by their word and example, provide to the youth of the parish.


The faith formation of your child(ren) is a splendid responsibility. Family and our faith community are interwoven by our beliefs. We aspire to bring your child(ren) into a deeper relationship with God, family, friends and community.  


Your example of putting your faith into action daily by living a faithful Christian life and attending Mass helps your child(ren) know how to be a good disciple. We greatly admire your dedication. Thank you for inviting us to participate actively in helping your child(ren) grow in their Catholic faith. 


All classes will be held in the classrooms of the Cardinal Joseph Bernardin School.


Classes are offered on Mondays, at two class times.

Grades  K - 6    4:30 - 6:00 PM

Grades  K - 8     6:30 - 8:00 PM

(Grades 7 & 8 classes are held from 6:30-8:00 PM only)

2024-2025 re REGISTRATION FORMS/LINKS

- All necessary registration forms can be downloaded by clicking on the titled tab. 

- Once completed, your forms can be mailed or dropped off at our Church office, office hours are Monday-Friday, 9:00 am-1:00 pm/Sunday, 8:00 -1: 00 pm. Religious Education Office hours are Monday- Thursday 9:00 am - 1:00 pm.

Returned registration forms are numbered as they are received by our office and will be processed accordingly. 

Please note: Our Religious Education Office is closed for the month of July and reopens on Monday, August 5.

(Registrations can be returned to the parish office during this time period.)

    We ask that you complete all parts of the registration form, especially…

    Email address AND phone numbers

    (Please provide current email addresses. Emergency and weather closures will be sent through texts.)

    - A copy of a baptismal certificate for each student must accompany your registration form. Also include a First Eucharist certificate, if applicable.

    - Grades 2-7 students transferring from another parish must obtain a transfer form from the previous R.E Program / Catholic School.

    4:30-6:00 PM session open to Grades K-6 only!

    6:30-8:00 PM session open to Grades K-8

    PHOTO RELEASE STATEMENT

    We anticipate a great year with the students and will have many opportunities while they facilitate the prayer services, participate in their grade level retreat, engage in Church tours, etc. and may be included in photos. We REQUIRE PARENTAL APPROVAL to publish these photos. We would like to be able to share these great happenings with the parish through pictures in the bulletin or on bulletin boards, Facebook and our parish website.

    Please “√” MAY versus MAY NOT be used.

    If left blank, we will assume that you do not want us to use your child’s photos in any form of media. No student’s name will ever be published.

  • MEDICAL FORM

    When completing your MEDICAL FORM, please write your family name across the top and include additional last names, if different than the Parent/Guardian.

    -Please list any allergies, medical conditions or current medications.


    For students who currently have an IEP/504 Plan on file at their district school’s office:

    - please "check" the box in the column listed IEP / 504 to alert us.

    - Please “check” if you request an aide for your student.

    - We will send a Student Accommodations Form thru the mail in August 2023 for you to complete. The student accommodations form allows you to share some ideas, concepts and strategies that are unique to your student.)

    - We DO NOT need a copy of the IEP / 504 Plan on file in the RE Office.   

    All information on the accommodations form that you provide is kept confidential; this information allows us to provide the best experience for your student. 

    - Your child(ren)’s Catechist will receive a copy of the accommodations form to help achieve the greatest potential success in the classroom. If you feel that your student would best be accommodated by having an individual assist them with classroom work while on site; please note this and we will do everything possible to provide a Faith Companion for him/her.

    PLEASE NOTE: 

    - Do not hold onto the forms because of payment issues. We appreciate full payment of the tuition & fees but understand that a down payment may be necessary at this time. We ask that all balances are paid by August 22, 2024.

    - Payments are accepted by cash, check (payable to St. Elizabeth Seton Religious Education) or via electronic payment. Electronic payments are accepted on the site GiveCentral, a very secure site used by the Archdiocese of Chicago. To make your payment electronically, please use our payment portal tab.
